Responsibilities

• Support Social Workers at Chris Hani Baragwanath Academic Hospital
• Screen patients' socio-economic circumstances and refer to community resources
• Provide basic counselling and support to patients and families
• Assist with intake, referrals, follow-ups, discharge planning and family tracing
