Z83 Form Filler
Every South African government post requires the Z83 application for employment form. This fills in the genuine gazetted form — the 2021 revised Z83 (81/971431) published by the DPSA — so what you download is the official document with your details on it, not a lookalike. Complete Section A fresh for each post you apply for.
Section B asks for your ID number, population group, gender, disability and criminal-record disclosures. None of it is saved to your FirstJobly profile or our database — it stays in this browser tab and is gone when you close it.
The Z83 replaces a covering letter, but you still attach a detailed CV. Only certified copies requested by the advert need to go with it.

Have you been convicted or found guilty of a criminal offence (including an admission of guilt)?
Do you have any pending criminal case against you?
Have you ever been dismissed for misconduct from the Public Service?
Do you have any pending disciplinary case against you?
Have you resigned from a recent job pending any disciplinary proceeding against you?
Have you been discharged or retired from the Public Service on grounds of ill-health, or on condition that you cannot be re-employed?
Are you conducting business with the State, or a director of a company doing business with the State?
If employed in the Public Service, will you immediately relinquish those business interests?
Only if your occupation requires registration (SACE, SANC, ECSA, SAICA…).
C. Contact details and medium of communication
One per line — email, phone number and postal address.
D. South African official language proficiency
Rate how you speak and how you write or read each language. Up to five languages fit on the form.
E. Formal qualifications
Highest first. Four rows fit on the official form — list the rest on a separate attachment.
F. Work experience
Most recent first. Three rows fit on the form — your attached CV carries the rest.
If you were previously employed in the Public Service, is there any condition that prevents your re-appointment?
G. References
Real people who have agreed to be contacted during office hours. Departments do phone them.
Declaration and signature
Every Z83 must be signed, and each page initialled — an unsigned form is disqualified during screening.
Before you choose how to sign
Departments differ on digital signatures. Many accept a signature drawn on screen, but some insist on a wet-ink signature and will disqualify a form signed electronically. Check the advert and, if it says nothing, the safest route is to print the form and sign it by hand.
Departments require each page of the Z83 to be initialled.
